Mental Health Screening

Elementary School Students

Virtual appointments for students Grades PreK-6 can be scheduled for Thursday, November 5.

Thank you for your interest in participating in Davis County’s Mental Health Screening Nights for Youth. These events are a free opportunity for youth of all ages.  A parent must participate with their student.  A separate registration is required for each child that will be participating.
Virtual appointments for students Grades PreK-6 can be scheduled for Thursday, November 5.
Virtual appointments for students Grades 7-12 can be scheduled for Wednesday, November 4.
An online screening survey will be sent by email a few days in advance of your appointment and should be completed prior to your online meeting with a clinician. The screening will address social and emotional needs and strengths, depression, anxiety, trauma, and suicide.  The screening is not a full psychological evaluation and a clinical diagnosis will not be made.
At the appointment, youth and their parents will meet with a mental health professional to discuss screening results and talk about what follow-up is needed.
